Step 3 Overview: The credentialing process is the essential bridge between your lactation expertise and the families who need your care. By becoming an in-network provider, you eliminate financial barriers and establish your practice within the insurance ecosystem.

Foundations of Credentialing

Credentialing is where insurance companies verify your qualifications to ensure you meet their quality standards. Once complete, you can sign a contract and begin billing members at their in-network rates.

Application Readiness Checklist

  • [ ] Current IBCLC Certification through IBLCE
  • [ ] Professional Liability (Malpractice) Insurance certificate
  • [ ] IRS CP575 or LTR 147C form (EIN verification)
  • [ ] Formally completed W-9
  • [ ] Detailed CV with month/year for all positions (no gaps)
  • [ ] State license (if dual-licensed as RN/NP)
  • [ ] Business bank account info for EFT

Pro Tip: Start your credentialing applications as early as possible. The 90–120 day processing clock doesn't start until every document is perfect. And for lactation providers, payors rarely meet the typical timelines.

Substep: NPI & Taxonomy Management

Type 1 vs Type 2: You MUST have an Individual NPI (Type 1). If you have a business entity (LLC/Corp), you should also obtain an Organizational NPI (Type 2).

Taxonomy Codes: For IBCLCs, the primary code is 174N00000X. Ensure your primary taxonomy reflects your specialty to avoid rejections.

Substep: CAQH & Data Springs Profile

Your CAQH ProView profile is the 'Source of Truth' for insurance carriers. Upload your documents, grant 'Global' access, and re-attest every 120 days to avoid termination.

Signature Action: Keep your CAQH/DataSprings profile consistent. Any mismatch between your CAQH data and your application will cause immediate delays.

Substep: Individual vs Group & Choosing Carriers

Prioritizing Carriers: Look at your local market share. We recommend starting with Aetna and United Healthcare as they historically offer the best lactation coverage.

Pro Tip: Focus on the 2–3 biggest payers in your local area first to build your revenue stream quickly without overwhelming your tracking system.

Post-Enrollment Success Tasks
  • Tracking: Log Submission Dates and Reference Numbers. Follow up every 30 days.
  • EFT/ERA Setup: Enroll for Electronic Funds Transfer immediately to ensure payments are deposited.
  • Test Claims: Submit one test claim upon receiving your Welcome Letter to verify your profile.

Signature Action: Never assume an approval is active until you verify the effective date and run a successful test claim. Tracking call logs prevents lost applications.
