Check VFC Eligibility at Every Vaccine Visit
A pediatric appointment does not establish program eligibility
A pediatric practice sees a child for preventive care and plans vaccination. Before selecting vaccine inventory and building the claim, the team needs to determine whether the child qualifies for the Vaccines for Children program for that encounter.
CDC identifies VFC eligibility for children age 18 or younger in specified categories, including Medicaid eligibility or enrollment, lack of insurance, American Indian or Alaska Native status, and qualifying underinsurance. VFC providers must screen and record eligibility at each visit. Underinsured access is limited to specified settings or approved deputization arrangements. These are program rules, not a finding that every child with an unpaid insurance claim qualifies. [1]
A prior eligibility entry should not replace the required review at the current visit.
Resolve the vaccine source before administration
Make eligibility screening part of the vaccine workflow rather than leaving it for the biller after the encounter. Keep the required screening record and make the result available to the staff selecting stock.
For example, a child's insurance situation may change between well visits. The team should review the current answers and program requirements before assuming the same vaccine source applies.
Use the state or territorial immunization program for uncertain cases, especially questions about underinsurance and approved locations. Do not treat a deductible, denied claim, or delayed eligibility response as a substitute for reviewing the applicable category. The clinical team should address timely care while staff resolve the administrative question.
Separate the vaccine product from administration
CDC states that a VFC provider cannot charge for vaccine supplied through public sources. It also explains that a VFC provider needs Medicaid enrollment to receive Medicaid payment for administration services provided to Medicaid-eligible children. Product supply and payment for administering the vaccine are different parts of the arrangement. [1]
Confirm the state's reporting requirements for publicly supplied vaccine and the administration service before configuring claim defaults. A privately purchased vaccine workflow should not automatically populate every VFC encounter.
Explain the charges that may be associated with the visit under the applicable program and plan. Avoid calling the entire appointment free simply because the vaccine product came through VFC. Review any patient-charge restrictions before collecting payment.
Reconcile inventory, the administration record, and the submitted claim. The vaccine source in the record should agree with the billing treatment. A reliable eligibility check helps the practice use the correct stock and keeps families from being caught between a program benefit and an inaccurate bill.
