Phase 1: Research State Rules
The first step is a deep dive into your specific state's Medicaid handbook. Look for specific language regarding 'Lactation Services' and whether IBCLCs are recognized as independent providers or must bill under a supervising physician.
Phase 2: Verify Provider Eligibility
Determine if your national certification and NPI are sufficient for enrollment. Some states may require additional state-level licensure or specific background checks before you can apply for a Medicaid provider ID.
Phase 3: Managed Care vs. Fee-for-Service
Map out whether you will bill the state Medicaid agency directly (Fee-for-Service) or need to contract with private Managed Care Organizations (MCOs). Each MCO may have its own credentialing timeline and reimbursement rates.
