Temporary NP Coverage Needs Its Own Billing Plan

Coverage on the schedule is not a billing exception

An independent practice needs temporary clinical coverage. The regular clinician is away, patients still need appointments, and a nurse practitioner is available. Calling the arrangement locum tenens does not settle whose NPI belongs on the claim.

Medicare's fee-for-time compensation rules allow specific substitution arrangements. Noridian expressly states that services of nonphysician practitioners, including NPs, may not be billed under those fee-for-time or reciprocal billing reassignment exceptions. Its guidance also describes a limited physical therapist exception. This is not a general permission for any covering clinician to use someone else's NPI. [1]

Set up the coverage before patients arrive

Check the temporary NP's individual enrollment, group relationship where applicable, and eligibility for the actual payer and service location. Ask the payer how it handles temporary NPs rather than assuming a physician substitution policy includes them.

Document who examined the patient, who made the decisions, and who completed the service. A staffing agency's contract can establish a business relationship, but it does not rewrite the insurer's claim requirements.

If the NP is not ready to bill, changing the rendering identity is not an administrative shortcut. Resolve the enrollment or contracting issue and develop a lawful coverage plan before the practice promises patients in-network appointments.

Keep other billing pathways separate

Do not confuse substitution with incident-to billing. They are different concepts with different requirements. A physician being somewhere in the organization does not establish that an NP's visit qualifies for another billing pathway.

Commercial contracts may address temporary coverage differently from Medicare. Obtain the applicable written policy and keep the response with the coverage plan. A representative's general assurance that the practice is in network does not answer whether the covering NP is properly included.

Temporary staffing should preserve patient access without making the claim describe a clinician who did not provide the service under the rules being used.
