Your Appointment Slot Does Not Determine the Psychotherapy Code

Bill the service time that occurred

A calendar may reserve an hour for therapy. That does not establish an hour of psychotherapy. Late arrival, an early ending, or administrative interruptions can change the time that supports the claim.

A Medicare contractor billing article lists 16 through 37 minutes for 90832, 38 through 52 minutes for 90834, and 53 minutes or more for 90837. The article is contractor guidance, not a promise of payment from every insurer. Apply the current code rules and the policy for the actual payer. [1]

Make the record support the time

Use a documentation method that reliably captures actual psychotherapy time. Distinguish the clinical service from scheduling, payment collection, or another service that has its own billing rules. Do not let the scheduling system automatically choose a code before the clinician has confirmed the work performed.

For example, a patient scheduled for a longer visit may receive only 45 minutes of psychotherapy after arriving late. The reserved appointment length does not supply the missing minutes. Conversely, a clinically necessary longer session should be documented accurately, not shortened on the claim simply because the calendar used a standard slot.

Time alone is not the whole record. The note should explain the treatment provided, its connection to the patient's needs, and the plan. A timestamp cannot substitute for clinical documentation.

Review the workflow before the audit

Check whether templates prefill a duration, whether the biller receives the actual time, and whether a clinician's correction reaches the claim before submission. A convenient default becomes a problem when nobody verifies it.

Also distinguish standalone psychotherapy from psychotherapy furnished with an eligible prescriber's evaluation and management service. Counselors should not borrow a prescriber's coding structure because it appears on a payer's list.

Patients do not need a lecture about coding thresholds at every visit. They do need a practice whose claims consistently describe the care they actually received.
