A Doula Support Note Should Describe Support Without Inventing Medical Care

Document the work within the role

A doula can provide substantial help during pregnancy, birth, and postpartum recovery without diagnosing a condition or providing medical treatment. The note should explain that support clearly. It should not borrow clinical language simply because the service is being submitted to insurance.

California's Medi-Cal guidance describes covered doula services as nonmedical support and identifies a benefit effective January 1, 2023. That is a specific state program, not evidence that every insurer covers the same services or that doula certification creates a clinical license. [1]

Replace vague language with observable work

A note saying patient treated for anxiety may describe a service outside the doula role if it implies clinical diagnosis or psychotherapy. A more accurate record would describe the support actually offered, such as discussing the parent's stated concerns, helping prepare questions for the medical team, or providing agreed comfort measures.

Record the service date, the support provided, and how it related to the client's needs. Include the program's required elements when billing a benefit. Do not add an assessment or claim a treatment outcome that did not occur.

If the doula also holds a nursing or other clinical license, make clear which role and service the appointment involved. Possessing another credential does not make every doula encounter a separately billable clinical service.

Make referrals without claiming to diagnose

Build a clear process for directing medical questions or concerning symptoms to the appropriate licensed clinician. Document the communication accurately. A recommendation to contact the medical team is not the same as having ruled out a complication.

Explain the scope to clients before services begin, including what the doula does, what remains with the clinical team, and how urgent concerns will be handled. Review the applicable payer's requirements rather than copying another state's documentation template.

A good support note does not need to sound like an office medical visit. It needs to show the real work and preserve the boundary between supportive care and licensed clinical care.
